T Level Student Completion Declaration Form – Printable Templates

Printable Student Completion Declaration form templates (formerly Annex H) for colleges that need to complete and store physical records alongside other T Level placement documentation.

Overview

These printable templates allow colleges to record a student's industry placement completion in a format that can be signed, stored, and shared outside of Grofar. They are consistent with DfE industry placement completion requirements.

DfE guidance update (May 2026): The DfE has removed the annex labels from its T Level documentation. The completion declaration is now called the Student completion declaration form. The template content and what it covers are unchanged. If your team uses the old name, "Annex H" still refers to the same form.
